The following are procedures in the application of the Ayushman Bharat Yojana (PM-JAY) card, otherwise referred to as the Ayushman Card.

Locate an Enrolment Center: Find an Ayushman Bharat Yojana Sachetna Kendra or Common Service Center (CSC) in your locality, a designated site where enrolment is done.
Gather Documents: Bring the necessary documents, such as a piece of ID (Aadhaar card, voter ID, or ration card), a piece of residence, and a piece of income (when needed).
Visit the Enrolment Center: Visit the Sachetna Kendra or CSC along with your required documents and show your interest in enrolling in the Ayushman Bharat Yojana.
Fill the Enrolment Form: The enrolment form can be completed manually or via an online portal by the operator in the center.
Provide Family Details: Family members. Decent information about members of your family, their names, age, and relationship with the head of the household.
Verify Eligibility: The operator can also demand extra documents or explanations to confirm your qualifications.
Receive Confirmation: A slip of confirmation or a confirmation of receipt will be sent to the candidates after enrolment is complete.
Card Delivery: The Ayushman Card will be dispatched and delivered to the registered address within a couple of weeks.
It is important to remember that the Ayushman Card is provided to the family as a whole, and the card is to be printed with his or her personal information. The card is renewable and applicable for a given time, which is mostly one year, and must be renewed after a certain duration.
